Given the context of the lyric, I don't think thats at all filthy. In a different context yes, definitely! The word "come" is clearly being used to indicate "move from where you are to where I am", not "ejaculate semen from your penis".

Firstly that's not in the middle, it's right at the end, and secondly, it's not really a hidden double meaning is it...... it has a blatent
meaning stuck right at the end, be it rude or not.

Possibly! I wouldn't worry about it though. Madonna great writer? Hmmm - she's a great performer, and a great showman (show-woman??!), with a knack for tapping the popular cultural influences over time. All that adds up to a useable talent, and she's made the most of it though. But great writer? But I know you are a massive fan, and I certainly respect that, and I would never argue that she hasn't been incredibly successful.

Eating out = it could mean Madonna could have been in a cafe, or it could mean something else. She said once in an interview with Jonathan Ross she meant it to be taken either way. O shock, horror!! Balls did ask!! So I explained!
